Learning Outcomes:
Understanding of the role of the nurse in meeting care needs of patients with long- term health conditions, applying knowledge relating to assessment data, in order to plan safe, appropriate and individualised care.
To be able to formulate appropriate plans of care for patients with long-term conditions, using integrated care planning and/or care pathways, recognising the importance of the roles of others in multi-professional care.
To be able to analyse the use of national service frameworks, government policies, self-management and current evidence which informs the management of patients with long- term conditions.
To be able to apply a legal and ethical framework and a non-discriminatory approach to analysing situations from practice.
